Office Policies

  • Optimized Health & Hormones specializes in weight loss, nutrition/dietetics, hormone balance, and peptides. I am available by appointment only and have no coverage after hours. However, all patients will have access to texting services in case of urgent issues or concerns. Please allow up to 24 hours for response.

  • I request that all patients keep a primary care provider as a partner in their healthcare. Your insurance network usually chooses these providers, and I will collaborate with them to provide you with the best care. I do not provide primary care services.

  • Optimized Health & Hormones is not in network with insurance companies. Upon your request, I will provide you with a receipt of service. This can be used to file for insurance reimbursement. Please keep a copy of this receipt for your records before sending the original to your insurance company. I do not get involved in any disputes between you and your insurance nor do I bill directly for any treatments or consultations.

  • For most prescribed medications, I work with compounding pharmacies. Compounded medications are typically not covered by insurance.

  • Letters of medical necessity are provided under special circumstances. Please allow 48 hours for a letter to be formally written and emailed.

  • In an effort to minimize my environmental impact, I make every effort to be paper free. Documents and receipts are managed online and will be emailed to you.

  • I do not overbook. A time slot is reserved especially for you when you schedule an appointment. If you need to cancel or to reschedule an appointment, please provide 24 hours advance notice.

  • Payment is due at the time of service. Cash, check, or credit card payments are accepted. Please note: Credit card payments are assessed a 3.75% processing fee.

  • I charge for my time as other professionals do. My fee schedule is as follows:

    • New patient consultations are $150. Please allow 45 minutes for your initial visit.

    • Phone visits are $75. They are available if you are an established patient and are limited to 15 minutes. They are not considered a comprehensive office visit.

    • Hormone pellet insertions are $375 per procedure. The cost of the pellets (estradiol and testosterone) is included in this fee.

    • Weight loss follow ups with medication prescriptions/management are $150.

    • Lab reviews are $75. They can be completed over the phone or in office.

  • Refunds for services previously rendered are not permitted.
